This is an agreement between South Tampa Puppy Palace (STPP) and the pet owner whose name appears below. It contains a medical power of at- torney for your pet, a waiver of certain legal rights and a full release of all liability arising from services provided by STPP.

Please read carefully before signing.

1) The signer of this agreement represents that he/she is the sole owner of the pets named above and , as owner, agrees to be fully responsible for all the terms and representations contained in this agreement. The owner further agrees to pay for any and all pet care services rendered plus any other charges that may arise as described in this agreement. Payment for these charges are due and payable upon the departure of your pet. The owner acknowledges that their pet shall not leave the STPP until all charges are paid in full.

2) Owner understands and agrees that STPP is relying on their representations contained in the PET PROFILE (contract exhibit A) and affirmatively states that their pet is in good health and has not been ill with any communicable condition within the last 30 days. Owner also warrants without reservation that their pet(s) has not harmed or shown any aggressive behavior towards any person or other pet.

NOTE: South Tampa Puppy Palace reserves the right to deny admittance to any pet at any time for any reason. Pets attending doggie daycare must be spayed or neutered.

3) Owner certifies that their pet(s) is current on ALL VACCINATIONS. Dogs must have DHLPP (Distemper, Hepatitis, Leptospira, Parainfluenza, Parvovirus), Bordatella (kennel cough), and Rabies Vaccinations'. Cats must be current on their Rabies & FVRCP.While boarding, if pet is found to have fleas or ticks, owner authorizes STPP to give flea/tick bath at owners expense Owner agrees to disclose, on an ongoing basis, any medical condition or behavioral problem that arises affecting their pet's potential suitability to stay at STPP.

NOTE: The foregoing vaccinations must be verified by a licensed veterinarian prior to your pet's first stay and must stay current at all times. Please have your vet fax or È-mail these records before arrival. Sorry, we do not accept vaccinations done by breeders.

4) Owner hereby acknowledges that pets are unpredictable in nature. Group play is not for pets that have temperament problems such as aggression or extreme shyness. Although activities are supervised, whenever dogs play together, there is a possibility of injury from rough play. SPP provides off leash socialization in doggie day care and I understand injuries to my pet might occur at any time. I assume and accept responsibility for all potential injury to my dog including but not limited to bites, scratches, bodily injury, illness, disease, theft, loss, or death. Should an injury or loss of any kind occur during the pet's use of the facilities, the owner agrees not to bring any liability charge of any type against STPP. Furthermore, I agree STPP will not be held responsible for any injuries, escape or death due to fire, acts of nature, or for any cause whatsoever. Initial

NOTE: Pet owner agrees to assume liability for their pet and to pay damage claims and expenses arising from their pet's actions toward another pet or person. In all circumstances pet owner agrees to hold SPP harmless and to indemnify STPP, its owners and its employees against any charges made by any third party due to the actions of owners pet at any time.

5) Owner accepts that there IS always the possibility of a GROOMING accident. Grooming equipment is sharp and despite professional precautions, a pet could be cut,scratched or otherwise injured. This usually only happens when a pet wiggles or jumps unexpectedly. Please advise us if you have a nervous pet so we can take additional precaution.

6) Owner agrees to allow SPP to make health care decisions for their pet during their absence provided no written instructions exist and all efforts to contact owner or emergency surrogate have failed. Should owner's pet be injured unexpectedly, or in the event of any emergency, the pet owner authorizes STPP (if owner or surrogate is unavailable) to engage the services of licensed veterinarian to give requisite attention to their pet at their sole expense Owner also agrees to indemnify STPP from any claims, charges or lawsuits (including reasonable attorney's fees) arising from services provided by any veterinarian for their pet.

NOTE: Owner agrees, upon demand, to reimburse South Tampa Puppy Palace for all veterinarian charges made on behalf of their pet.

7) Owner agrees to pay SPP for services rendered at the rate in effect at check in. All fees and any additional charges allowed by this agreement shall be paid at the time of pick up. No pet shall leave STPP (unless other arrangements have been made) until all charges are paid in full.

8) Boarding charges are charged by the night. Check out time is 6:30pm Monday thru Friday and 3:30 on Saturday. After these hours your dog must board for an additional nights at your expense.

9) Owner agrees that any controversy or claim arising out of or relating to this agreement shall be settled in accordance with the rules of the American Arbitration Assn. A judgement rendered by an arbitrator may be entered in any court having jurisdiction thereof. The arbitra- tor shall award the prevailing party the costs of such arbitration and reasonable attorney's fees. I understand that this agreement along with the STPP "Pet Profile" comprises the entire agreement between me (pet owner) and South Tampa Puppy Palace. Initial

10) This agreement shall be valid for a term of 12 months. After this period either party has the right to cancel with a 30 day written notice. If no notice is given this contract shall continue on a month to month basis until such notice is given.

I (pet owner) certify that I have read this entire agreement and that I understand the policies of STPP and agree to abide by them. I further state that I agree to all the terms set forth in this agreement.
